Use this service to arrange travel vaccinations and get advice for your upcoming trip abroad.
To allow sufficient time to assess your travel health needs and arrange any required vaccinations, travel risk assessment forms must be submitted at least 8 weeks before your departure date. We recommend submitting your form as early as possible once your travel plans are confirmed.
Late requests: Unfortunatley if your departure date is within 8 weeks, we will be unable to review your forms as we cannot guarantee availability of appointments or completion of all recommended vaccinations before your departure. In such cases, please attend a private travel clinic to ensure timely protection.
Before you start
We’ll ask you for:
- your first and last name, date of birth,address, email and phone number and your travel details
- if applicable, the details of the person you are completing the form on behalf of
